We are looking for an organised and motivated individual to join our practice team in a new and interesting role as Care Coordinator.
This is a new role in the primary care system, designed to help patients with more complex needs navigate their way through the healthcare system.
Clinical experience is not needed as all training will be provided in the post, but a positive attitude, flexibility and willingness to work within a team are essential.
Your role will include a mixture of administrational tasks and talking to patients, as well as assisting members of staff in the practice team with a wide range of duties.
You would be joining an enthusiastic and loyal practice team at the The Clements Surgery.
