We are looking for two Care Navigators to join our team at our Long Melford & Lavenham Practice sites, part of Unity Healthcare.
What’s the role?
- You’ll be the first point of contact for patients accessing our surgery
- You’ll help ensure our patients can access care at the right time, with the most appropriate clinician
- The aim is to reduce the reliance on GP time, reduce inappropriate referrals and avoid unnecessary hospital admissions.
- You’ll help enhance the care that our patients receive, by handling queries from patients in relation to appointments.
Unity Healthcare is run by Suffolk GP Federation CIC, a not-for-profit community interest company.

- You’d be suited to this role if you have a caring and empathetic nature
- It’s important that you can work well as part of a team as well as independently
- You’ll need to be able to pick up new IT systems with supportive training
Contract type
- Role 1 - 07:45 - 18:30 Wednesday and Friday one hour lunch,
- Role 2 07:45 - 18:30 Monday, Tuesday and Friday
