Are you looking for a new role?

We are seeking a Diabetes Specialist Nurse to join our team on a part time, permanent basis. The role involves managing complex diabetic cases, providing education and promoting self-management to improve patient outcomes. Working closely with the wider primary care team, the Diabetic Specialist Nurse will ensure that high-quality, evidence-based care is delivered, following best practices in diabetes management.

This is an excellent opportunity for a local Practice Nurse who has a specialist interest in Diabetes Management and is committed to providing high-quality care in a primary care setting.

Qualifications

  • Registered Nurse with valid NMC registration
  • Postgraduate certification or diploma in Diabetes Care
  • Extensive experience in managing diabetes care, including complex cases such as insulin initiating and adjustment
  • Strong understanding of diabetes pathophysiology, treatment options, and lifestyle interventions for managing the condition
  • Excellent communication skills to educate and support patients, families and other healthcare professionals
  • Ability to work independently and as part of a multidisciplinary team
  • Experience in primary care or community settings.

Desirable

  • Mentor/ teaching qualification. Experience of teaching or mentoring
  • Independent and/or supplementary nurse prescribing qualification
