Care Navigator / Receptionist
We are looking for a Care Navigator / Receptionist to join our team based at Unity Healthcare, Haverhill.
What is the role?
- Handle enquiries from patients and carers in relation to appointments with the practice clinical
team.
- Proactively manage available appointments with practice staff on a day-to-day basis, ensuring
that clinical time is preserved and protected and that patients with the most urgent needs are
prioritised.
- Offer advice to patients and carers, in order to manage healthcare needs through a problem solving approach; e.g. directing to the appropriate clinician, signposting to appropriate services (e.g. equipment and adaptations which meet peoples’ needs and reduce risk where possible), connecting people with the appropriate support services in the community (e.g. pharmacies or voluntary services)

You’ll join our practice, Unity Healthcare based in Haverhill which is run by Suffolk GP Federation. We are a not-for-profit, community interest company and our main purpose is to support Primary Care across Suffolk, including support with working collaboratively and at scale. We also deliver a portfolio of NHS services across Suffolk and North East Essex, including Podiatry, Ultrasound, Diabetes, as well as Urgent Care Services including the Out of Hours face-to-face Service in Suffolk.

- It would be advantage if you have previous experience of dealing with customers or patients via telephone or email.
Contract type:
We are looking for 2 people to join us on a permanent part time basis as follows:
Position 1 Monday 13:30-18:30, Tuesday 13:30-18:30 Wednesday 07:45-18:30.
Position 2 Tuesday and Thursday 07:45-18:30
